Chronic Lung Conditions

Chronic lung conditions, including COPD, emphysema, and chronic bronchitis, bring many challenges for both the patient and their family. If left unmanaged, these conditions can be both physically and emotionally demanding, and their symptoms can adversely affect the patient’s quality of life. 

Our Chronic Lung Conditions Patient Program helps patients regulate their symptoms and significantly improve their overall wellbeing. 

Our program involves: 

  • Identifying and reducing risk factors, such as smoking and environmental irritants 
  • Infection prevention measures, including flu and pneumonia vaccinations

Stroke Management

Suffering a stroke can physically, mentally, and socially affect an individual and make it challenging for them to regain a sense of normalcyOur Stroke Management: Patient Program utilizes a multidisciplinary approach that addresses all aspects of a patient’s life that has been affected by a stroke. Our program is specially designed to help patients adapt to new life abilities and reach a maximum level of independence.

  • Identifying and reducing risk factors, such as smoking or uncontrolled diabetes 
  • Medication management that includes VNA pill box to help keep patients organized and on-track 
  • Physical, occupational and speech therapies 
  • Medical and physical tools, including blood pressure cuff and exercise pedaling device 
  • Follow-up care 
  • Educational tools and materials to help prevent another stroke 
  • Encouragement, motivation and inspiration 

Fall Prevention 

Unintentional falls can be difficult to predict, and even more difficult to recover from. We offer a Fall Prevention Program that assists patients with specific challenges that may increase their susceptibility to a fall. The program also helps patients understand the numerous factors that may contribute to their falls before they happen. Our Fall Prevention Program includes: 

  • Identifying and monitoring risk factors such as vision changes, medication side effects, or potential hazards in the home 
  • Medical and physical tools, including an exercise peddler and blood pressure cuff 
  • Nursing and therapy services to improve mobility, coordination and independence
