It’s not uncommon for family members who are caring for a loved one at home to find themselves handling more than just their loved one’s immediate health care needs. Oftentimes, family members must manage many aspects of their loved one’s day-to-day life, including financial obligations, doctor’s appointments, caregiving scheduling, medication refills, and so much more.

Our care management team consists of qualified registered nurses who assist family members with managing their loved one’s health conditions and associated responsibilities, including:

Handling and monitoring basic financial transactions
Assisting with transitions to and from doctor appointments, outpatient procedures and hospital stays
Coordinating home health care services

Providing referrals for elder law, financial planning, bill paying and more

In addition to assessing, planning, coordinating and managing your loved one’s health and life needs, our care management nurses can coordinate private, in-home visits with our home health aides (HHAs). Our HHAs are available for both hourly shifts and 24-hour care, and can assist with:

Bathing, grooming, and dressing
Light housekeeping duties
Meal preparation
Companionship and social interaction
Transportation to and from medical appointments
