Duties and Responsibilities:

  • Provides quality and safe delivery of home health care services by implementing current standards of practice.
  • Establishes a plan of care and clinical pathway based on physician’s orders.
  • Develops reasonable, specific and measurable goals that commensurate with plan of care.
  • Evaluates patients by conducting a thorough assessment, applying diagnostic and prognostic muscle, joint, nerve and functional abilities tests.
  • Instructs the patient/caregiver and other health team members in the care and use of therapeutic equipment and devices.

Summary
