We are looking for qualified LPN’s with previous Home Health Experience for work in St. Lucie, Martin and
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Provides quality and safe delivery of home health care services by implementing current standards of nursing practice; demonstrates competency in necessary clinical skills; understands the home health coverage guidelines for Medicare/Insurance; is responsible for following physicians orders; is responsible for reducing any Verbal Order to writing (the day it is given) to be reviewed by the Supervisor or Case Manager and signed by the physician; counsels and instructs patients/families in order to accomplish goals of care; assesses the patient's condition and reports significant changes to the physician, supervisor and other team members. Assures that written progress reports are made to the physicians as needed; and other duties as directed.
