- High cholesterol
- Diabetes
- Sedentary lifestyle
- Diet high in fat and sodium
Tracking medications
- Follow doctor’s orders
- Discuss any possible side effects
- Keep a list of all medications you are taking
Receiving tools to help progress
- Blood pressure cuff
- Pillbox
- Exercise peddler
- Self-care handbook
This program will help the patient keep the rehabilitation process on track with a guiding hand and clinician’s eye.
